Here is your chance to Honor a Teacher at MHS!

As another school year winds down, no doubt you’ve met a teacher, administrator or staff member who has challenged, encouraged or inspired you or your child.

This is your chance to acknowledge that person while contributing to the Madison Education Foundation.

SENIOR STUDENTS & PARENTS:
Don't leave MHS behind without leaving something behind!

Please take a moment to reflect on your MHS years and recognize any teachers, administrators and staff who you feel were particularly helpful to you.
|
The teacher, administrator or staff member you honor is sure to smile when receiving the recognition certificate with your own words of gratitude (see example below) and a silk rose during the last week of school.
To participate, please print out and complete the Honor a Teacher Donation Form. If honoring multiple people complete separate forms for each one and combine all the tribute donations into one check. Forms are also available in the MHS office.
Mail completed form(s) together with your check to: MEF PO Box 1093 Madison, NJ 07940
MEF is a 501(c)(3) organization. Your gift is tax deductable.
Orders for tributes received by June 4 will be placed in teachers’ mailboxes during the final week of school. Orders received after the June 4 deadline may not be processed in time for the last day of school, which may result in the teacher not receiving their certificate until September.
